Step 2

Gather Evidence of Your Disability

The NDIS requires clear evidence of your disability and its impact on your daily life. This can include:

  • Evidence of your diagnosis
  • Evidence that your disability is permanent and life-long
  • Treatment history including what hasn’t been effective in the past
  • Evidence of the functional impact your disability has on your daily life including your support needs to complete daily tasks. This is best demonstrated through an Occupational Therapy
    Functional Capacity Assessment and Report.
  • Any further allied health assessments (e.g., occupational therapy, psychology, speech pathology).

Step 3

Complete the NDIS Access Request

You’ll need to submit an Access Request Form to the NDIS, along with your supporting evidence.

Your local office can be found here: https://www.ndis.gov.au/contact/locations

Step 5
