1. Reflect on Your Current Plan

  • Are you achieving your NDIS goals, or have your needs changed?

2. Gather Evidence and Reports

  • Collect recent reports from therapists, support workers, or service providers.
  • Request progress updates and recommendations for future supports.

3. Track Your Progress and Challenges

  • Keep notes or a diary of achievements, difficulties, and unmet needs.
